- $10.50 FLAT fee, per order, regardless of size, quantity, weight, etc., for Medical Supplies
- Additional $10.50/CS for food items (Boost, Ensure, etc.); if order ONLY contains food item, the FLAT $10.50 for Medical Supplies is NOT assessed in addition to the per case freight
- Additional $10.50/CS for incontinence products; if order ONLY contains incontinence products, the FLAT $10.50 for Medical Supplies is NOT assessed in addition to the per case freight
- Additional FLAT fee of $13.95 for lightweight DME equipment (walkers, rollators, canes, commodes, crutches etc.) regardless of quantity; if order ONLY contains lightweight DME equipment, the FLAT $10.50 for Medical Supplies is NOT assessed in addition to the FLAT $13.95
- Additional FLAT $28.95 for wheelchairs regardless of quantity; if order ONLY contains wheelchairs, the FLAT $10.50 for Medical Supplies is NOT assessed in addition to the FLAT $28.95
